Now offering the COACH Program

National Diabetes Week 2023 aimed to kick start Australia’s biggest conversations about the impact of diabetes in Australia – conversations to drive change, and to create hope for the future. ABS data 2021, showed that every 8 minutes, an Australian is diagnosed with Diabetes and if you are born overseas, you are more likely to have or get diabetes.

To better support the communities and keep the conversations going, the Chronic Disease Program is now offering The COACH Program for Multicultural Communities. It is a free program supporting community members diagnosed with Chronic Disease/s and living within Metro North to achieve better health and live longer though self-advocacy, self-management, education, and support. The program is delivered by Multicultural Health Workers in language (including but not limited to): Arabic, Burmese, Mandarin, Vietnamese, Cantonese, Dari, Hindi, Malayalam, Swahili, Samoan, English or via an interpreter at a place and time most appropriate for the community members.
