My Health for Life Program for Samoan Community

In partnership with Goodna Neighbourhood Centre, ECCQ Chronic Disease Program is delivering a My Health for Life Program to our Samoan community residing in local area. My health for life is a free, four-month program where you work with a health coach to achieve your health goal. It is funded by the Queensland Government and delivered by the Healthier Queensland Alliance including Diabetes Australia, ECCQ, Heart Foundation, Stroke Foundation, Aboriginal and Torres Strait Islander Health Council and Queensland Primary Health Networks. The program is designed to help Queenslanders stay well and lessen their risk of developing conditions such as type 2 diabetes, heart disease and stroke.

To help overcome some of our community members’ language barrier, the program is being delivered in Samoan by our qualified Multicultural Health Worker. We also offer the program in simple English and in many other languages such as Vietnamese, Mandarin, Cantonese, Farsi, Dari, Burmese, Sudanese Arabic and Arabic.

If you are interested in attending My health for life program in your local area or if you want to find out more on what we do at ECCQ Chronic Disease Team, find us on Facebook and Instagram, call us on 3844 9166 or email us on chronicdisease2@eccq.com.au.
