Chronic Diseases Program

ECCQ’s Chronic Disease Program has been delivering culturally appropriate preventative care since 2007. We provide flexible, healthy lifestyle education and resources to support the prevention and self-management of chronic diseases for CALD communities.

The Chronic Disease Program provides flexible, culturally appropriate healthy lifestyle education and resources to support the prevention and self-management of chronic diseases for CALD communities.

Our services are FREE and available through in-person sessions, by telephone or online platforms, and are delivered in languages including (but not limited to) Cantonese, Hindi, Dari, Burmese, Mandarin, Arabic, Vietnamese, Samoan and simple English.

Local Drug Action Team

Supports creating stronger family and greater community connections, and enhances knowledge on the consumption of alcohol and drugs.

Cancer Free Cultures

This program aims to empower communities in Queensland to improve cancer awareness, prevention, and access to screening services.

Multicultural Living Well Program is here to help you learn about your risk of developing a chronic disease, make healthier lifestyle choices and connect you with the Australian Health Care System.

My Health for Life

This initiative has been culturally tailored and is delivered in-language by a qualified Health Coaches to support CALD communities to make healthier lifestyle choices.

A program that helps you, your family, and your community make healthy lifestyle choices, manage your diabetes, heart disease, asthma, lung disease, and chronic kidney disease, and improve your health!

The ECCQ Chronic Disease Program offers free group education sessions designed to empower communities with essential health knowledge.
These interactive sessions cover vital topics, including health screening, navigating the Australian healthcare system, and managing conditions such as heart disease, diabetes, chronic kidney disease, and lung diseases. Participants also learn about physical activity, reducing smoking and alcohol consumption, and strategies for living well with chronic conditions. Sessions are tailored to suit beginner, intermediate, or advanced English levels, ensuring accessibility for diverse communities.

We have published a number of chronic disease resources, including cookbooks, booklets on healthy eating and cooking, and chronic disease awareness and prevention.

These come in simple English and several languages translations, including: Arabic, Samoan, Burmese, Tongan, Swahili, Vietnamese, Traditional Chinese, Simplified Chinese, Dari, Punjabi, Hindi and Tamil.
